Pet Information which allows to you record personal and identifying data about your pet.
The first page of the book leads you
through the basics of Disaster Preparedness and the things you can
do to help protect your pets. This list is by no means all-inclusive, but
gives you an overview of things you need to think about and prepare for in
case of an emergency or natural disaster.
The Home Check Up page
walks you through a weekly go-over with your pet to catch any potential
problems before they become serious.
The Names and Numbers section provides two forms. One for Vet and
Insurance information, another for names, address and phone numbers of
frequently used places such as pet sitter, dog walker, groomer, pet store and
emergency contact.
There are two Immunization charts, one
for Dogs and one for Cats. Use the one you need and discard the other.
The Meds and Illnesses section provides a chart to records
Preventive Medications such as Flea/Tick, Heartworm and Deworming
applications. There is also a page to record any illnesses and treatments
your pet has had.
Finally, there are 10 Vet Visit
pages to record all information about each visit to the Vet, such as date,
reason for visit, questions for the doctor, tests/results, medications
etc.
